True Gynecomastia vs Pseudogynecomastia

True gynecomastia involves actual glandular breast tissue under the nipple — firm, often tender, and resistant to diet or exercise. Pseudogynecomastia is fat accumulation in the chest without glandular tissue. Most patients have a mix of both. The distinction matters because only surgery can remove glandular tissue; fat-only cases may respond to liposuction alone.

The Surgical Technique

The most common approach combines liposuction to remove the fatty component with direct gland excision through a small peri-areolar incision. The gland excision removes the firm disc of tissue directly behind the nipple. In cases with significant skin excess (Grade 3–4), additional skin removal may be needed, resulting in a longer scar.

Causes and Risk of Recurrence

Common causes include puberty, obesity, anabolic steroids, SARMs, certain medications, and hormonal imbalances. If the underlying cause is not addressed, tissue can regrow. Steroid or SARM use must be stopped before surgery and ideally long-term to maintain results. Weight-related pseudogynecomastia can return with weight gain.
